Insulin Delivery and Storage in Pakistan's Heat: A Cold-Chain Guide
Insulin is a protein that loses strength when it gets too hot or freezes. In Pakistan, where summer temperatures often pass 40 °C, how insulin is transported and stored at home matters as much as which insulin you use. This guide is prepared by the Medicine Online pharmacy team under the medical oversight of Dr. Farhan Rashid Abbasi and is general information, not a substitute for your doctor's advice.
Storage rules that apply to most insulins
- Unopened insulin: keep in the refrigerator at 2–8 °C, in the main body of the fridge, not the freezer or right against the cooling plate.
- Never freeze insulin. Insulin that has been frozen must not be used, even after it thaws.
- In-use pen or vial: can usually be kept at room temperature (below 25–30 °C, depending on the product) for a limited period, often about 4 weeks. Some insulins, such as Toujeo, last longer once opened. Always check the leaflet for your product.
- Keep it out of sunlight and hot cars. A car parked in the sun can exceed 60 °C within minutes.
- During load-shedding: keep the fridge door closed. For long outages, an insulated pouch with a cool pack (not ice directly on the insulin) helps.
How to check insulin when it arrives
- The parcel should contain an insulated box or pouch with cool packs.
- The insulin should feel cool, not warm and not frozen.
- Clear insulins (such as Lantus, NovoRapid or Humulin R) should look clear and colourless. Cloudy insulins (such as Mixtard 30, Humulin N or Insulatard) should mix evenly when gently rolled, with no clumps or crystals.
- Check the expiry date and that the seal is intact.
- Put it in the fridge straight away.
If anything looks wrong, do not use it. Send us photos within 48 hours and we will replace it under our refund policy.
How Medicine Online ships insulin
- Insulin is packed in insulated packaging with cool packs, kept refrigerated until dispatch.
- Delivery usually takes 1–2 business days to major cities and 3–5 business days elsewhere. In summer, choose an address where someone can receive the parcel promptly.
- Delivery is free on orders above Rs 15,000; a flat Rs 450 applies otherwise. Cash on Delivery is available in Sindh.
You can also order insulin syringes and a reusable NovoPen 4 in the same order.
Frequently asked questions
Can insulin be delivered safely by courier in summer?
Yes, when it is shipped in insulated packaging with cool packs and delivered within a few days. Refrigerate it as soon as it arrives.
My insulin was left out of the fridge overnight. Is it still usable?
If it stayed below about 25–30 °C and was not in sunlight, it is usually still usable, but its in-use period starts counting. If it got hot or froze, replace it. Ask your pharmacist if you are unsure.
Should I keep my in-use pen in the fridge?
Many people keep the pen they are using at room temperature because cold insulin can sting. Follow the leaflet for your specific insulin.
