Vitamin D Deficiency in Pakistan: Symptoms, Testing and Supplements
It surprises many people that a sunny country has so much vitamin D deficiency, but studies across Pakistan have found low vitamin D in a large share of adults and children. Indoor lifestyles, covered clothing, air pollution, darker skin and diets low in fortified foods all play a part. This guide is prepared by the Medicine Online pharmacy team under the medical oversight of Dr. Farhan Rashid Abbasi.
Why vitamin D matters
Vitamin D helps the body absorb calcium and keeps bones and muscles strong. Long-term deficiency can cause rickets in children and softening of the bones (osteomalacia) in adults, and contributes to osteoporosis and falls in older people.
Common symptoms
- Tiredness and low energy
- Aches in the bones, back and legs
- Muscle weakness or cramps
- In children: delayed walking, bowed legs or slow growth
Many people have no symptoms at all, which is why testing helps.
Getting tested
The blood test is called 25-hydroxy vitamin D. Labs report it in ng/mL. As a rough guide, many labs use under 20 ng/mL as deficient and 20–30 as insufficient. Your doctor interprets the result alongside your symptoms and calcium levels.
Types of vitamin D supplements
| Type | Examples | Typically used for |
|---|---|---|
| Drops | D-Max drops 400 IU | Babies and young children |
| Daily or weekly capsules and chewables | D-Max 2000 IU chewable, Sunny D 5000 IU, Sunny D 10000 IU | Maintenance or treatment as advised |
| High-dose "stat" doses | Sunny D Stat 200,000 IU, Sunny D Insta injection | Treating confirmed deficiency, on a doctor's advice only |
| Calcium with vitamin D | Osnate-D, Qalsium-D, CAC-1000 Plus | When calcium intake is also low |
Do not overdo high doses
Vitamin D is stored in the body, so repeating 200,000 IU doses without testing can push levels too high and raise calcium in the blood. This can cause nausea, thirst, confusion and kidney stones. Take high-dose vitamin D only as prescribed, and recheck your level after treatment.
Tips to make supplements work
- Take vitamin D with a meal that contains some fat, which improves absorption.
- Get 15–20 minutes of sun on the arms and face a few times a week where possible, outside the hottest hours.
- Eat eggs, oily fish and fortified milk where available.
- Babies who are breastfed are often advised a daily vitamin D drop; ask your paediatrician.

Frequently asked questions
How long does it take to correct vitamin D deficiency?
Usually 8–12 weeks of treatment, followed by a lower maintenance dose. Symptoms such as aches often improve within a few weeks.
Is the 200,000 IU injection better than capsules?
Not necessarily. Oral vitamin D is well absorbed in most people. Injections are mainly used when someone cannot absorb or take oral doses.
